Getting started with Open Grace is simple. Your IRC Service Coordinator initiates the process and we handle the rest.
Your IRC Service Coordinator identifies Open Grace LLC as a CFS provider and submits a referral through the Regional Center. Families can also request a referral at their next IPP meeting.
Director Joshua Kennedy personally reaches out to your family — by phone or text — to introduce Open Grace, answer questions, and schedule a no-pressure intake conversation at your convenience.
Once authorized through your IPP, your dedicated Family Coordinator begins working alongside your family — building trust, navigating systems, and providing support tailored to your goals.
CFS is flexible and individualized. Most families have multiple needs at the same time — Open Grace addresses them holistically.
We make the referral process simple and we follow up promptly with every family you send our way.
Review the family's IPP and confirm CFS authorization. Open Grace accepts referrals for benefits navigation, care coordination, independent living, long-term planning, and all other CFS service areas.
