Your IRC Service Coordinator identifies Open Grace LLC as a CFS provider and submits a referral through the Regional Center. Families can also request a referral at their next IPP meeting.

Referring a Family to Open Grace

We make the referral process simple and we follow up promptly with every family you send our way.

Identify the family's CFS needs

Review the family's IPP and confirm CFS authorization. Open Grace accepts referrals for benefits navigation, care coordination, independent living, long-term planning, and all other CFS service areas.

Call or email Joshua to discuss the referral, confirm availability, and share any background that would help us serve the family well from day one.

We reach out to the family within 48 hours

Joshua personally contacts the family to introduce Open Grace, answer questions, and schedule an intake conversation — no pressure, no rush.

Once authorized, services begin immediately. You will receive regular progress updates and all documentation is maintained in full IRC compliance.

What is Coordinated Family Support (CFS)?

CFS is a Regional Center service that provides individualized support to adults with developmental disabilities who live at home with their family. The program focuses on increasing independence, supporting caregivers, and connecting families to the services and resources they need. Services are flexible and tailored to each family's unique situation and IPP goals.

Who qualifies for CFS services?

To receive CFS services the individual must:

  • Be 18 years of age or older
  • Have a developmental disability
  • Live at home with family or caregivers
  • Be eligible through a Regional Center such as Inland Counties Regional Center (IRC)
  • Have CFS authorized in their current Individual Program Plan (IPP)

Does CFS replace services like IHSS or ABA?

No. CFS does not replace or duplicate any existing services. Instead CFS helps your family get approved for services you may not have yet, ensures your existing services are working together effectively, and fills the gaps that other services do not cover. Think of Open Grace as the coordinator that brings everything together for your family.

Yes. Open Grace provides culturally appropriate, family-centered services and can support families who primarily speak Spanish. We can facilitate communication with providers and agencies to ensure your family fully understands the services and supports available to you.

Can Open Grace come to appointments or agency visits with us?

Yes. When appropriate Open Grace Family Coordinators may support your family during appointments by helping explain information, advocating for your loved one, and ensuring your concerns are heard and addressed by the agency or provider.

Can CFS help with non-verbal individuals or complex needs?

Yes. Open Grace can help obtain communication tools such as AAC devices and PECS, coordinate specialized services, train caregivers and providers, and provide support tailored to individuals with complex medical or behavioral needs. Every individual deserves support that meets them where they are.
